Harbour Hospice provides specialist palliative care for patients and families, alongside the care provided by their GP and specialist teams.
Is it for me?
Consider accepting a referral to Harbour Hospice when you are still reasonably well but you have some needs that require assessment (e.g. pain or symptoms or needing support at home). This way, you will be able to have better quality of life and feel supported during your illness.
Anyone with a life-limiting condition may be referred to Hospice for care and support. Patients are usually referred to Hospice through a healthcare professional, such as a GP or medical specialist.
It is possible to refer yourself or your family member. If you would like to talk to someone about accessing our services please contact us or you can email us directly on referral@harbourhospice.org.nz.

If you are a healthcare professional who would like to refer a patient to us please click here.
Referrals can now be made directly from most GP practices via Medtech and MyPractice programmes. This is a fast and easy way to refer, and allows you to directly attach the most up to date patient notes and results

If your request is for a patient to be seen within 24 hours please contact the teams direct by phone so we can prioritise support of your referral. After hours please request to speak to the Harbour Hospice doctor on call
