Harbour Hospice provides specialist palliative care for patients and families, alongside the care provided by their GP and specialist teams. 

 

Is it for me?

Consider accepting a referral to Harbour Hospice when you are still reasonably well but you have some needs that require assessment (e.g. pain or other symptoms, or needing support at home). This way, you will be able to enjoy better quality of life and feel supported during your illness.

Anyone with a life-limiting condition may be referred to Hospice for care and support, with referrals most often made by a healthcare professional, such as a GP or medical specialist.

It is also possible to refer yourself or your family member. If you would like to talk to someone about accessing our services please contact us, or email us directly at referral@harbourhospice.org.nz.

Our team will link you to a Harbour Hospice health professional to discuss your needs.

If you are a healthcare professional who would like to refer a patient to us, please click here.

Referrals can now be made directly from most GP practices via Medtech and MyPractice programmes. This is a fast and easy way to refer, and allows you to attach the most up to date patient notes and results.
